Premature Ejaculation: Causes, Treatments and How to Regain Control

November 16, 2022

What is premature ejaculation?

Premature ejaculation (PE) is a male sexual dysfunction characterized by ejaculation that occurs before or shortly after penetration, with minimal sexual stimulation, and without the person wanting it. This condition can cause frustration, stress and affect both a man's self-esteem and his relationship.

Although it can occasionally happen to any man, it is considered a disorder when it occurs recurrently and persistently for at least six months.

Common Causes

Premature ejaculation can have physical or psychological origins, or a combination of both. Common causes include:

Psychological Factors:

  • Anxiety related to sexual performance
  • Depression or generalized stress
  • Negative sexual experiences in the past
  • Relationship problems or lack of communication

Biological Factors:

  • Abnormal levels of neurotransmitters (such as serotonin)
  • Hyperthyroidism
  • Inflammation or infection of the prostate or urethra
  • Penile hypersensitivity

Types of premature ejaculation

  1. Primary (lifetime): it occurs from the first sexual experience and is almost always present.
  2. Secondary (acquired)- develops after a period of trouble-free sexual activity.

How is it diagnosed?

The diagnosis of PE is made based on the patient's medical and sexual history, in addition to a physical evaluation if necessary. Sometimes it measures the intravaginal latency time (IELT), which is the time from penetration to ejaculation. An IELT of less than 1 minute usually indicates EP.

Effective treatments

1. Psychological and sexual therapies

  • Cognitive Behavioral Therapy (CBT): helps identify and modify negative thoughts about sex.
  • Couples therapy: improves communication and reduces emotional pressure.
  • Control techniques such as “stop and start” or “pressure” (Masters and Johnson method).

2. Medications

  • SSRI antidepressants (such as paroxetine, sertraline, or fluoxetine): increase the time until ejaculation.
  • Topical anesthetics (creams or sprays): reduce penile sensitivity.
  • Dapoxetine: specific medication for PE in some countries.

3. Exercises and lifestyle changes

  • Kegel exercises: strengthen the muscles of the pelvic floor.
  • Reducing stress: with meditation, yoga or physical activity.
  • Avoid excessive consumption of alcohol or tobacco, which can make the problem worse.

Practical tips for regaining control

  • Don't feel alone or embarrassed: It is a common and treatable condition.
  • Talk openly with your partner: This can reduce emotional pressure.
  • Be patient: Treatments can take time and practice.
  • Seek professional support: A sexologist or urologist can guide you properly.

Conclusion

Premature ejaculation doesn't have to define your sex life. With the right combination of medical treatment, psychological support and communication as a couple, it is possible to regain control and enjoy a full and satisfying intimate life.
